New patients should arrive 30 minutes prior to their scheduled appointment to provide adequate time for registration.
In case questions arise, it's a good idea to bring along any proof of insurance, information on any medications you may be taking, or other information that might be related to your medical care.
At each visit you will check in with one of our specially trained patient account representatives who will verify and update your demographic information including address, phone numbers, insurance information and any other special billing requirements for that visit. Registration forms may be updated on a periodic basis. Please download a hard copy of our registration form, which you can print and bring into the office to save time.
